If you have been receiving care as an inpatient, we provide on-going support once you have been discharged from hospital, helping you to re-establish your normal level of function.
When your mental health deteriorates in outpatients, requiring you to have increased support at home but you are not unwell enough to need hospital treatment, we liaise with you and your existing clinical team to identify the most suitable level of community care.
If you do not have an outpatient team, we can introduce a suitable clinical service during your treatment with BREVIN, that can continue once you no longer need us.
